How do I use the Questionnaire Editor?

The Questionnaire Editor allows you to create custom questionnaires with different question types, response options, and scoring mechanisms.

1. From the Editors menu on the top right, select Questionnaires to open Questionnaire Editor.

2. On the Questionnaires page, click on New on the top right to create a new Questionnaire.

3. Fill in required fields for Questionnaire Properties.

4. Add and configure Questionnaire Items as needed.

5. Add and configure Variables for the Questionnaire

6. Set up HTML Report Template or PDF Report Template

7. Save and / or Finalize the Questionnaire.
